How it works

A plan people can follow, and someone checking in

  1. Before they leave

    We'll turn the treating team's instructions into a plain-language plan: what happened, what's changed, what to do each day and when to get help. Plans are designed to start from templates approved by cardiologists, with the treating team signing off each one.

  2. The first weeks at home

    Short check-ins keep the plan on track and ask people how they're going. In an emergency, we tell them to call Triple Zero (000).

  3. Reporting for the fund

    Funds will see who took part and what happened next, compared with a matched group of members who didn't take part.
